Home Modifications

Access II, Independent Living Center provides limited home modifications and adaptive equipment to individuals with substantial physical disabilities as funding is available.  Funding comes from private donations and grants.  Co-pays may be required.  Program guidelines are as follows:

  • This service is provided to individuals who receive Access II ILC’s Consumer Directed or In-Home Services.  Access II ILC will work with the consumer, other agencies and individuals to coordinate modifications on a cost-sharing basis.  If additional funds are available after Access II ILC consumers’ and cost-sharing consumers’ needs are met, Access II ILC will work to meet the needs of individuals on the waiting list.
  • The individual must have a total household income at or below 185% (FPL) Federal Poverty Level (all income within the household must be considered, including child support).
  • The individual must have no other means of obtaining the needed home modification or adaptive equipment.  Other means could include; family, VA assistance, CDBG, Rural Housing, insurance.
  • If rented/leased, the tenant must obtain written permission of the owner to make the modifications.
  • The maximum amount available per project paid by Access II will be $1000.00 Funding limit may be increased as deemed necessary by the approval of Executive Director or Access II ILC Board of Directors.

 



  • The consumer will be responsible for a 10% co-pay of amount available ($1000). All costs exceeding the amount available ($1000) per project will be the full responsibility of the consumer. A consumer that cannot meet the co-pay costs will be considered for service on a case-by-case basis and may be waived, as deemed necessary by the approval of the Executive Director or Access II ILC Board of Directors.
  • The consumer must have less than $5000 total in savings, retirement, annuity or other accounts.
  • If the modification is under $300, length of residence requirement will be at least 6 months at current. If renting, the individual must be current on rent paid and not have an eviction action pending.
  • If the modification is over $300, length of residence requirement will be 6 months or more at the current residence. If renting, the individual must be current on rent paid and not have an eviction action pending.
  • All work will be awarded by competitive bid, and all requests must be accompanied with at least two (2) bids meeting ADA guidelines.  In the provision that labor costs exceed $600.00 in any one calendar year, the contractor must complete the Ramp/Home Modification Contractor Tax Information form.
  • The home must be in good enough condition to support the modifications. 
  • The individual must demonstrate a physical impairment or need for the equipment or modification.  A written physician’s notice of medical necessity is required. For home modifications, the proposed modification must be directly related to a physical disability.  If the individual is requesting a ramp, the physical disability must restrict their daily activities to the point that they use a wheelchair as their primary mode of mobility.  
  • Individuals are placed on a waiting list based on the severity of their needs. Individuals with an emergency need (ie: cannot return home from an institution without modification) may receive higher priority and may be served immediately if funds are available.
  • If Access II ILC has a targeted grant to provide home modifications or adaptive equipment, the grant guidelines will supersede the above guidelines. 
  • Consumers may receive 1 home modification per household per 10 years.
